3. DOCUMENTS REQUIRED FROM THE POTENTIAL EMPLOYERS 
apaska. Photocopy of NRIC (both husband and wife)
b. Marriage certificate
c. Birth Certificate of child or children
d. Latest J Form
e. Completed Employer Information form

4.TERMS AND CONDITIONS

CATEGORY 1 – EMPLOYER DID NOT OPT FOR INSURANCE SCHEME
1)  
Agency will replace run away maids within 3 months. However, employer shall be responsible for the following cost:-
a)
  Levy (new maid)
b)
FOMEMA (new maid)
c)
Personal Bond (for run away maid)
2) 
Agency will replace maids for poor performance within 3 months. Employer will bear the following costs:-
a)
Levy (new maid)
b)
FOMEMA (new maid)
c)
One way ticket to her own country (for existing maid)
3)
Agency will replace medically unfit maids within 3 months at no costs.
 
CATEGORY 2 – EMPLOYER OPT FOR INSURANCE SCHEME
1)
Agency will replace maids who run away within 6 months at no cost to employer.
2)
Agency will replace maids for poor performance within 3 months. Employer will bear the following costs:-
a) 
Levy (for new maid)
b)
FOMEMA (for new maid)
c) 
One way ticket to her own country (for existing maid)
d) 
Insurance (for new maid)
3) 
Agency will replace medically unfit maids within 3 months at no cost.
4)
When maids run away after 6 months and before 12 months the Agency shall reimburse the insurance claim to the employer provided the employer return all relevant document required for insurance claim (original police report, original penalty receipt for RM250)
     

*     For Category 1 & 2, the total salary held by the employer shall be handed over to
      the Agency

**   For Category 1 & 2, the Agency shall be only liable for 1 replacement.
